Assumption of Risk

I understand that participation in baseball training, lessons, camps, strength training, open gym activities, and general use of the facility involves inherent risks.

These risks include, but are not limited to:

• Being struck by baseballs, bats, or training equipment

• Use of pitching machines

• Multiple hitting stations operating at the same time

• Deflected or ricocheted balls leaving nets or cages

• Collisions with other participants

• Slipping, tripping, or falling on turf, flooring, or equipment

• Physical exertion, strength training, and conditioning activities

• General movement within an active baseball training environment

I understand that injuries may occur as a result of these activities. I voluntarily assume all risks associated with participation at The Strike Zone 3.

Facility Rules and Safety Acknowledgement

The participant and parent/guardian acknowledge and agree to the following safety rules:

• No running in non-designated areas

• No swinging bats outside of designated hitting areas

• Helmets must be worn when required

• Only authorized staff may operate pitching machines

• No horseplay or unsafe behavior

• Follow all coach and staff instructions at all times

• Remain aware of surroundings in an active training environment

Failure to follow facility rules may result in removal from the premises without refund.

Release of Liability

In consideration for being allowed to participate in activities at The Strike Zone 3, I, on behalf of myself and the above-named minor, hereby release, waive, discharge, and hold harmless:

The Strike Zone 3, its owners, officers, employees, coaches, trainers, contractors, and representatives

from any and all claims, demands, causes of action, damages, or liability arising out of or related to participation in activities at the facility, including but not limited to claims arising from negligence.

I agree to indemnify and hold harmless The Strike Zone 3 from any claims resulting from the participant’s actions while at the facility.

Medial Authorization

In the event of an injury or medical emergency, I authorize The Strike Zone 3 staff to obtain emergency medical treatment for my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.

Photo and Video Release (Optional)

I grant permission for [Facility Name] to use photographs or video recordings of my child for promotional purposes, including website content, social media, and marketing materials.

Code of Conduct

The participant agrees to:

• Show respect to coaches, staff, and other athletes

• Refrain from bullying or inappropriate behavior

• Use equipment properly

• Respect the facility and property

Damage caused by intentional misuse of equipment may result in financial responsibility.

Acknowledgement and Signature

By signing below, I acknowledge that:

• I have read and understand this Release and Waiver of Liability.

• I understand that I am giving up certain legal rights.

• I voluntarily agree to the terms stated above.
